The call will last approximately 45 minutes, so having all the necessary information at your fingertips will ensure a smooth and efficient process. Here’s a checklist to help you get ready for your scheduled advisement:
Necessary Documentation
Spousal (or Parental) Coverage Information
If applicable, have your summary of benefits and the cost details to join your spouse’s or parent's health plan. This information will help you compare your options.
Social Security Number
Ensure you have the Social Security Number for each individual needing coverage. This is essential for processing your healthcare plan.
Proof of Residency
You'll need to provide proof of residency for each individual who is not a U.S. citizen or National. These documents or clear photos of them can be uploaded to your Remodel Health profile prior to your scheduled advisement.
Household Income Information
Gather all income-related documents for your household, such as pay stubs, W-2 forms, or wage and tax statements. Typically, you will need your Adjusted Gross Income (AGI) to assess your options accurately.
If you have a housing allowance or any other deduction, please also provide this information when updating your Remodel Health profile or bring it to your advisement and make sure your Advisor is made aware.
Prescriptions
Prepare a list of all prescriptions and dosages for each individual needing coverage. This will help your Advisor navigate which plans would be best for you to review and consider in an effort to best accommodate your medication needs.
Doctors
Compile the names of all your healthcare providers, including primary care physicians and specialists. Having this information ready will help your Advisor show you plan options that include your current doctors.
